1. I acknowledge that staff will make every effort to contact parents/guardians in the case of a medical emergency. If I cannot be reached, I authorize camp staff to seek appropriate medical care.
2. I understand that camp staff may not administer any medications or sun screens.
3. I will remit timely payment according to the payment plan I indicated on my registration form, page.1.
4. I understand that CFH Ministries, COHOP and camp staff are not responsible for lost or stolen items and that my child should not bring personal belongings such as toys, jewelry, money, hand-held games, or Travel-size music devices (like I-pods/MP3 players).
5. I will make sure my child will be dressed appropriately for camp and will wear athletic shoes or sneakers.
6. I will drop off and pick up my child at the designated time. I understand that I will be required to show a picture ID, as will anyone I authorize to pick up my child.DISCIPLINE:
Campers will be expected to adhere to the rules of camp. Praise and positive reinforcement will be used to promote positive and understanding interactions. The camp will follow the following Discipline Procedures:
1. Quiet reprimand/verbal warning.
2. After repeated behavior problems, the camper may be temporarily removed from the activity, and a first written or oral incident report will be given to parent.
3. Additional behavior problems will constitute a second written or oral incident report given to parent, a parent meeting, and a possible 1-2day suspension from program.
4. If problems persists, a third written or oral incident report will constitute that the participant might be asked to leave the program. No refund will be given in the case of camper expulsion due to discipline infractions.
5. Campers may be subject to immediate expulsion in case of severe offenses. No refunds will be given.
